Lavaca Family Health Clinic


The Rural Health Clinic is located inside the Professional Building at Lavaca Medical Center. Patient care in the clinic is provided by 2 Family Practice physicians, 5 Mid-Level Staff members (Family Nurse Practitioners), and 6 Specialists (1 Gastroenterologist, 2 Podiatrists, and 2 Cardiologists).

The Family Practice physicians and Mid-Level Staff provide medical care to patients of all ages, from pediatric to senior.

Lavaca Family Health Clinic - Chronic Care Management


For Medicare patients:

If you have 2 or more chronic conditions, CCM (Chronic Care Management) services can help you manage your overall healthcare easier, so you can spend more time doing what you love! The primary goal of this program is to help patient's achieve a better quality of life through ongoing care and management of their chronic conditions.

Examples of chronic conditions include, but are not limited to, the following:

Alzheimer's disease and related dementia
Arthritis (osteoarthritis and rheumatoid)
Asthma
Atrial fibrillation
Autism spectrum disorders
Cancer
Cardiovascular disease
Chronic Obstructive Pulmonary Disease (COPD)
Depression
Diabetes
Hypertension
Infectious diseases such as HIV/AIDS

Services include:

At least 20 minutes a month of chronic care management services via phone.
Personalized assistance from a dedicated healthcare professional who will work with you to create your     care plan.
Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more.
Phone check-ins between office visits to keep you on track.
24/7 emergency access to a healthcare professional.
Expert assistance with setting and meeting your health goals.
